Large cavities and gradually worsening fractures over time are signs that a tooth may have a non-vital pulp. Intense toothaches, especially at night, are typical indicators of pulp inflammation. Therefore, whenever a patient detects cavities or experiences prolonged or intense pain, it is advisable to seek prompt treatment. Delaying treatment can complicate the restoration process, requiring more time, effort, and expense.

The absence of current pain does not necessarily mean the tooth does not need treatment. Past intense toothaches, even if currently absent, may indicate that the pulp has suffered damage. Therefore, the absence of pain does not eliminate the need for treatment.
